RENTER RESPONSIBILITY
The renter assumes full responsibility for the rented device from delivery until timely tender to FedEx using the return label issued by TWVC. The device may be used only at the approved location by properly trained persons and may not be transferred, altered, opened, repaired, pledged, or loaned without written approval. Loss, theft, damage, malfunction, relocation, contamination, or any safety incident must be reported immediately.
RETURN DEADLINE AND HOLDOVER RENT
Contact TWVC promptly for the authorized return label. Requesting a label is not a return. The complete equipment must be packaged and tendered to FedEx. The renter has five calendar days after the scheduled rental end date to obtain the first FedEx possession scan. If no scan occurs by the end of day five, holdover rent begins on day six at the daily rate shown on the rental invoice and continues until the first FedEx possession scan. TWVC may waive a delay caused by TWVC or the carrier. If the equipment remains unreturned for 30 days, or the renter refuses to return it after written demand, TWVC may charge the $8,500 replacement value plus accrued rent and documented related costs.
CLEAN AND COMPLETE RETURN
Return the device wiped visibly clean and free of blood, body fluids, drainage, food, dirt, adhesive, residue, hair, odor, infestation, waste, sharps, and other debris or contamination. The return must include the device, clean carrying bag, power supply and cord, and every return-required item listed on the packing record. A visibly dirty device or bag is subject to a minimum $150 cleaning fee. Extraordinary repair, biohazard or decontamination work, missing accessories, and additional shipping are charged at documented actual cost. Unused consumables are not credited unless TWVC agrees in writing.
INSPECTION AND EVIDENCE
TWVC may photograph and inspect the equipment at shipment and return. Return acceptance and any charges are based on the device condition, completeness, shipment records, photographs, and inspection. If replacement value is charged and the device is later recovered, TWVC will credit the amount required to prevent duplicate recovery, less accrued rent and documented costs.
CLINICAL USE
TWVC supplies equipment and operational support but does not diagnose, prescribe, or direct patient care. The treating clinician remains responsible for clinical decisions, orders, monitoring, and safe use.
